Stroke Rehabilitation

Each year in the UK, around 100,000 people experience a stroke, making it a leading cause of disability and death. Strokes occur when the blood supply to the brain is disrupted or blocked, leading to a lack of oxygen and nutrients in brain tissue.

There are three main types of stroke:

  • Ischaemic stroke – caused by a blood clot blocking a vessel in the brain, accounting for around 85% of strokes.
  • Haemorrhagic stroke – caused by bleeding in or around the brain, often due to a burst blood vessel.
  • Transient Ischaemic Attack (TIA) – a temporary blockage, sometimes called a mini-stroke, which increases the risk of a major stroke.

Following a stroke, people may experience weakness, stiffness, and spasms, affecting mobility and daily activities. Over a third of stroke survivors face communication difficulties, and fatigue can significantly impact recovery. 1 in 4 stroke survivors are of working age, yet many feel their support needs are unmet. While some recover quickly in acute stroke units, others require specialist neurological rehabilitation to rebuild skills and adjust to long-term challenges.

The aim of neurorehabilitation following a stroke is to help people achieve the highest level of independence and quality of life possible, by improving or restoring their abilities and supporting them to compensate for functional disabilities.

Our Approach to Stroke Rehabilitation

At Hamberley Neurocare, we take a structured, evidence-based approach to stroke rehabilitation, ensuring that every individual receives specialist, goal-focused support tailored to their unique needs. Our consultant-led services align with National Institute for Health and Care Excellence (NICE) guidelines [NG236], which recommend a minimum of three hours of therapy per day, five days a week, for those able and willing to participate.

Our interdisciplinary team includes:

  • Consultants in Rehabilitation Medicine
  • Neuropsychologists and Clinical Psychologists
  • Physiotherapists for postural management and muscle strength exercises
  • Speech and Language Therapists (SLTs) to support communication and swallowing difficulties
  • Occupational Therapists (OTs) to assist with mobility, daily tasks, and adaptations
  • Nurses experienced in managing stroke recovery and neurological conditions
  • Dietitians to support nutritional needs and recovery
  • Wellbeing & Lifestyle Coaches to promote emotional well-being
  • Life Skills Specialists who embed therapy into daily activities for continuous rehabilitation

Each rehabilitation plan is built around the individual, addressing physical, cognitive, and psychological needs. Our approach focuses on:

  • Restoring lost skills through targeted therapy.
  • Teaching new ways to perform daily tasks to adapt to any lasting effects of the stroke.
  • Providing practical adaptations to support long-term recovery.
  • Offering emotional and psychological support to improve confidence and quality of life.

To ensure progress, we use evidence-based assessment tools, including the FIM+FAM (Functional Independence Measure & Functional Assessment Measure) and Goal Attainment Scaling (GAS), tracking meaningful improvements in mobility, communication, and independence.

From the first assessment to discharge planning, our goal is to help every individual achieve their best possible recovery and transition successfully to the next stage of their rehabilitation journey.

Our Stroke Rehabilitation Pathway

To ensure structured, measurable progress, our Stroke Rehabilitation Pathway guides individuals through key stages of recovery. The Pathway ensures a structured yet flexible approach, adapting to each person’s potential for recovery. By integrating medical expertise, therapy, and real-world preparation, we help individuals regain independence and confidence, ensuring a smoother transition back to daily life. This pathway provides a structured framework for stroke rehabilitation, but every recovery journey is unique, with therapy tailored to individual needs, progress, and goals.
